Secure Account Management

This is an example of a typical form you might use for your users/clients to request a secure account on your website.  Approval isn't automatic.  Approval is required by the site owner (in this case, me) before your account is active.  Notification is automatic by email.

Usually, an applicant is asked to agree to the terms of access since the information presumably is private in nature.

Project information is restricted to project team members who have authorization to access the project website.  If granted access, you agree to respect the confidentiality of the information contained on the site.

Any files downloaded to your local computer remain the property of the site owner and cannot be redistributed for any reason beyond the necessary communications to complete your responsibilities in the project.
